HOW TO QUALIFY(BEGIN SERVICE)

It is a requirement that the patient’s doctor/s refer the patient to Hospice of Salina OR any other doctor of the patient’s choice.

Hospice of Salina receives inquiry phone calls from many sources: patients themselves, their family or friends, other healthcare providers such as physician offices, hospitals, nursing homes, assisted living facilities, home health agencies and other providers. When Hospice of Salina receives a phone call, we recommend that the facility, family, patient or other entity contact the patient’s personal physician for a referral. The patient can be assessed for Hospice service. We then contact the patient and/or family to explain our services. The Hospice admission nurse will also contact the physician’s office for the following information:

At the time of admission, the hospice interdisciplinary team will complete a comprehensive assessment of the patient/family needs. Our hospice interdisciplinary team will then develop a plan of care specific to the individual needs of each patient/family.
