*two factors are critical in determining prognosis: ability to breathe and to a lesser extent ability to swallow.

*exam by a neurologist within 3 months of assessment for hospice eligibility helpful in prognosis.

1. Demonstrate critically impaired breathing capacity

OR

2. Demonstrate BOTH rapid progression of ALS and critical nutritional impairment as demonstrated by all characteristics within 12 months of initial hospice certification.

3. Demonstrate BOTH rapid progression of ALS and life-threatening complications as demonstrated by all characteristics within 12 months of initial hospice certification.
